Finding Details:
The member was found to have committed professional misconduct in that she:
- abused a client verbally and emotionally, contravened a standard of practice of the profession and engaged in disgraceful, dishonourable and unprofessional conduct with respect to her actions and comments towards two clients in March 2015;
- contravened a standard of practice of the profession and engaged in unprofessional conduct between June 2014 and January 2016 by leaving a health record in an unsecure area and failing to notice the record was missing or taking appropriate action with respect to the missing health record, refusing to attend a meeting arranged by the employer to discuss client care issues, failing to administer medications, administering medications to the wrong client, administering too much medication to a client without consulting their medication record, and failing to document administration of a medication in the client’s medication administration record;
- contravened a standard of practice of the profession and engaged in unprofessional and dishonourable conduct between July 2014 and June 2015 by refusing to complete her assigned duties, communicating inappropriately with her clients and employer and/or kicking a co-worker in the knee, failing to advise medical staff that clients had not received their medications and failing to report her medication errors to the on-call physician;
- contravened a standard of practice of the profession and engaged in disgraceful, dishonourable and unprofessional conduct by communicating inappropriately with her clients and by failing to monitor and treat clients for symptoms of overdose after she administered too much medication to them;
- failed to keep records as required, contravened the standards of practice of the profession and engaged in dishonourable and unprofessional conduct between July 2015 and January 2016 by failing to document administration of medication in clients’ medical administration records and failing to complete medication administration error reports;
- falsified a record relating to her practice, contravened the standards of practice of the profession and engaged in dishonourable and unprofessional conduct in January 2016 by altering an entry in a client’s medication administration record made by another nurse regarding her administration of medication to the client; and
- engaged in disgraceful, dishonourable and unprofessional conduct between approximately 2014 and 2017 by engaging in unprofessional written and/or verbal communications with representatives of her employer and/or the College of Nurses of Ontario.
