Finding Details:
The member was found to have committed professional misconduct between March 2014 and December 2017 with respect to seven patients in that she:
- contravened the standards of practice of the profession and engaged in disgraceful, dishonourable and unprofessional conduct by administering insulin contrary to a physician’s order; administering epinephrine by intravenous injection when it was to be administered by intramuscular injection; failing to obtain consent from a patient and/or a substitute decision-maker before applying restraints; administering heparin at an incorrect drip rate; administering heparin when dalteparin had been ordered; administering insulin subcutaneously when it was ordered intravenously; administering epinephrine to a patient by intravenous injection when it was to be administered by intramuscular injection; failing to obtain consent before applying restraints; and failing to listen to and act on the concerns of a colleague about the care she provided;
- contravened the standards of practice of the profession, failed to keep records as required and engaged in disgraceful, dishonourable and unprofessional conduct by failing to transcribe a change to the physician’s order for Lasix in a medication administration record; documenting you had administered epinephrine by intramuscular injection, when you had administered by intravenous injection; failing to document the care she provided to a patient including the application of restraints, the consideration of available alternatives to restraints before applying restraints, obtaining a physician’s order for the application of restraints prior to and following the application of restraints, and the ongoing need for restraints;
- contravened the standards of practice of the profession, failed to keep records as required, falsified records and engaged in disgraceful, dishonourable and unprofessional conduct by altering a medication administration record to indicate that she had not administered insulin to a patient when she had; and by documenting that she had administered epinephrine to a patient by intramuscular injection when she had administered it by intravenous injection; and
The member was also found to have committed professional misconduct in that she:
- contravened the standards of practice of the profession, misappropriated property from a workplace and engaged in disgraceful, dishonourable and unprofessional conduct in or around January 2016 to April 2016 by applying for and receiving short term disability benefits from one employer while she was employed by another;
- contravened the standards of practice of the profession and engaged in disgraceful, dishonourable and unprofessional conduct in March 2019 by providing false information about her history with CNO to a facility while applying for a nursing position and/or while employed at the facility as a Registered Nurse, and in June 2019, by drawing blood from a patient who did not have an order for blood to be drawn; and
- signed or issued, in her professional capacity, a document that she knew or ought to have known contained a false or misleading statement in March 2019 when she provided false information to a facility about her history with CNO on her application for a nursing position.

Outcome Details:
The Discipline Committee ordered that the member’s certificate of registration be suspended for seven months; required the member to appear before the panel to be reprimanded; and imposed the following terms, conditions and limitations on the member’s certificate of registration:
- Must successfully complete specified remedial activities
- Must inform employer(s) of results of discipline hearing
- Must inform College of all nursing employer(s) for a period of time
- May not engage in independent nursing practice for a period of time
- Must have nursing mentor for a period of time
