Finding Details:
- The member was found to have committed professional misconduct in that she:
- contravened the standards of practice of the profession, gave information about a client to a person other than the client or his or her authorized representative and engaged in unprofessional conduct by disclosing personal health and other confidential information regarding the identity and residence of one client in September 2015 and two other clients in March 2016, without the consent of the clients or other authorization;
- contravened the standards of practice of the profession, misappropriated property from a client and engaged in disgraceful, dishonourable and unprofessional conduct in March 2016 by falsifying a cheque to herself from a client, and in July and August 2016, by misappropriating Percocet intended for a client;
- contravened the standards of practice of the profession, failed to keep records as required, falsified a record relating to her practice, and engaged in disgraceful, dishonourable and unprofessional conduct in July and August 2016 by falsifying and altering the narcotic count record and falsifying and entering the incorrect quantity of Percocet administered to a client in the medication administration record;
- contravened the standards of practice of the profession and engaged in disgraceful, dishonourable and unprofessional conduct in July and August 2016 by administering more Percocet to a client than had been authorized by a physician;
- contravened a term, condition or limitation on her certificate of registration and engaged in disgraceful, dishonourable and unprofessional conduct with respect to failing to comply with her interim order of the Inquiries, Complaints and Reports Committee between May and August 2016 by accessing and administering controlled substances, failing to notify her employer that the question of her capacity had been referred to the Fitness to Practise Committee and had not yet been decided, and failing to notify the College of Nurses of Ontario regarding her place of employment and the signed statement from her employer confirming that the employer received the required notification regarding the referral to the Fitness to Practise Committee.
