It is alleged that the member committed professional misconduct in that they:
-
contravened a term, condition or limitation of their certificate of registration and engaged in disgraceful, dishonourable or unprofessional conduct by failing to report charges relating to any offence to the Executive Director of CNO, when they failed to report that they were criminally charged with committing assault on three individuals in or about June 2016, contrary to Section 266 of the Criminal Code; entering the dwelling of an individual with the intent to commit an indictable offence therein in or about June 2016, contrary to Section 349(1) of the Criminal Code; committing mischief by wilfully urinating on the floor of a facility in or about June 2016, contrary to Section 430(4) of the Criminal Code; committing public mischief when they unlawfully caused a police officer and/or a peace officer to enter upon an investigation on two occasions contrary to Section 140(1)(c) of the Criminal Code, by reporting false bomb threats in or about December 2018 and/or by reporting that the offence of assault had been committed when it had not been committed in or about March 2020; two counts of repeatedly communicating with an individual, knowing that an individual is harassed or being reckless as to whether the individual is harassed, causing the individual to fear for their safety in or about March 2020, contrary to Section 264(2)(b) of the Criminal Code; and making repeated telephone calls (approximately 300) to a facility with intent to harass the facility in or about December 2018, contrary to Section 372(3) of the Criminal Code;

signed or issued in their professional capacity, a document that they knew or ought to have known contained a false or misleading statement and engaged in disgraceful, dishonourable or unprofessional conduct in or about January 2020, by submitting an employment application and a resume to a company that contained false information about their employment history and clinical experience, and omitting relevant information about their employment history; and/or
-
engaged in disgraceful, dishonourable or unprofessional conduct by phoning a former co-worker repeatedly and/or making harassing and/or threatening comments to them between October and November 2015; phoning and sending text messages to a co-worker/manager repeatedly and/or making harassing and/or threatening comments to them in or about October 2015; telling representative(s) of a former employer that they intended to report that the employer had engaged in fraud, for the purpose of obtaining a financial settlement in or about October 2015; slapping and/or threatening an individual with a knife in or about March to July 2016; stealing the individual’s insulin pump and/or charging the individual money to retrieve their belongings from their residence in or about March to July 2016; in respect of a psychoeducational assessment report regarding the individual: obtaining and taking a copy of the report without the individual’s consent and discussing the contents of the report with the individual’s mother without the individual’s consent in or about March 2016 to July 2016, faxing a copy of the report to the individual’s health providers without the individual’s consent, impersonating the author of the report when she faxed a copy of the report and included commentary about the report to the individual’s health providers, and/or making derogatory and inappropriate comments in a fax they sent to the individual’s health providers in or about July 2016; authoring and sending an email to the individual’s health provider which they represented as being authored by the individual in or about October 2018; accessing company email accounts of seven of their co-workers/former co-workers, approximately 139 times, without their knowledge or consent and without authorization in or about March to May 2020; accessing company systems and listening to telehealth calls involving approximately seventeen patients in or about April to May 2020 without authorization and/or accessing the personal health information of these patients without a clinical purpose for doing so and/or without authorization; falsely representing that they were an investigator employed at CNO in a call with a company employee and/or doing so for the purpose of obtaining information about a former co-worker in or about May 2020; making an audio recording of a telehealth call while accessing company systems without authorization and/or sending the audio recording of this telehealth call to the company by voicemail in or about May 2020; calling the company’s telehealth line on approximately three occasions for purposes other than to obtain medical advice and/or reporting medical concerns that they were not experiencing in or about May 2020; calling a company co-worker using the spoofed phone number of a second co-worker and impersonating a female in distress in or about May 2020; and/or contacting former company co-workers by phone, text and voicemail repeatedly in a harassing and/or threatening manner and/or making harassing and/or threatening comments to her co-workers in or about May and July 2020.
