Finding Details:
The member was found to have committed professional misconduct in that the member:
- falsified a record relating to the member’s practice; failed to keep records as required; contravened the standards of practice of the profession; and engaged in dishonourable and unprofessional conduct between January 2019 and April 2020, by failing to administer medications to 11 patients on 15 occasions but documenting that the member had administered them; in January 2019, by documenting that the member had administered Gravol to a patient in the patient’s MAR when the member had administered Diphenhydramine, and documenting an order for Diphenhydramine for the patient in the Facility’s Drug Record Book when the order was for Gravol; and in April 2020, by documenting that the member had applied a Nitro patch to a patient but never applied the patch;
- failed to keep records as required; contravened the standards of practice of the profession; and engaged in dishonourable and unprofessional conduct in February 2020, by failing to appropriately assess and document with respect to a patient after the patient fell; in April 2020, by altering the count on the Facility’s narcotic count sheet in relation to a patient contrary to the Facility’s “Pharmacy Policy & Procedure”; and
- contravened the standards of practice of the profession; and engaged in dishonourable and unprofessional conduct in January 2019, by administering Diphenhydramine to a patient when the order was for Gravol; in July 2019, by administering more Tylenol #2 to a patient than was authorized by the medication order; and in March 2020, by making inappropriate remarks about a colleague, who was performing the Facility’s mandatory screening for Covid-19.

Upon consideration of the member’s signed Undertaking for permanent resignation, the Discipline Committee made an Order requiring the member to appear before the panel to be reprimanded.
