A panel of the Inquiries, Complaints and Reports Committee of the College of Massage Therapists of Ontario accepted an Undertaking between the member and the College of Massage Therapist of Ontario (CMTO).
Pursuant to the Undertaking, the member acknowledged that they resigned their certificate of registration with CMTO and agreed to never reapply for membership, registration, licensure or similar status with CMTO.

Finding Details:
The member was found to have committed professional misconduct in that she:
- contravened a term, condition or limitation on her certificate of registration and engaged in dishonourable and unprofessional conduct by failing to provide CNO with details of an investigation, inquiry or proceeding for professional misconduct that she was subject to by the College of Massage Therapists of Ontario, including between October 2012 and February 2014, for allegedly failing to keep records as required; between December 2017 and February 2019, for alleged inappropriate billing practice(s); and between February 2018 and February 2019, for allegedly burning and injuring a patient during laser hair removal in January 2018; and
- signed or issued, in her professional capacity, a document that she knew or ought to have known contained a false or misleading statement and engaged in dishonourable and unprofessional conduct from 2013 to 2019, by providing false and misleading information to CNO in her annual membership renewal forms for 2014 to 2020, in that she indicated she was employed by a physician when she was not.

The Discipline Committee ordered that the member’s certificate of registration be suspended for two months; required the member to appear before the panel to be reprimanded; and imposed the following terms, conditions and limitations on the member’s certificate of registration:
- Must successfully complete specified remedial activities
- Must inform employer(s) of results of discipline hearing
- Must inform College of all nursing employer(s) for a period of time
