Details:
It is alleged that the member committed professional misconduct at one facility from February 2020 to July 2021 in that the member:
- abused patients verbally, physically or emotionally, contravened the standards of practice of the profession and engaged in disgraceful, dishonourable or unprofessional conduct, by making inappropriate remarks to a patient in that the member stated “your pussy stinks” and/or “you are going to die if you don’t eat” or words to that effect; and/or forcing or attempting to force the patient to eat, including by placing food into the patient’s mouth; by forcing or attempting to force another patient to eat, including by placing food into the patient’s mouth; and/or by forcing or attempting to force a third patient to ingest liquid; and/or
- contravened the standards of practice of the profession and engaged in disgraceful, dishonourable or unprofessional conduct by stating to staff that patients with mobility issues are getting what they deserve because they were sinners earlier in their lives, or words to that effect; yelling at staff on one or more occasions and in the presence of residents and/or family members; administering medication to a patient in the patient’s coffee and leaving the medication unattended with the patient; delegating medication administration of oral medications to Personal Support Workers and/or preparing medications and placing them on patients’ meal trays for Personal Support Workers to administer; and/or failing to remain with patients while they ingested their medications.
It is alleged that the member committed professional misconduct at a second facility in or about November 2017 in that the member:
- contravened the standards of practice of the profession and engaged in disgraceful, dishonourable or unprofessional conduct by failing to complete a head injury routine for a patient who sustained an unwitnessed fall; and/or inappropriately throwing the patient’s head injury routine document in the garbage; and/or
- falsified records relating to the member’s practice, contravened the standards of practice of the profession and engaged in disgraceful, dishonourable or unprofessional conduct by falsely documenting that the member completed head injury routine for the patient on 8 occasions when the member had not completed one or more of the documented assessments; and falsifying a head injury routine document for the patient in that the member transcribed assessments completed by other nurses and forged their signatures.
It is alleged that the member committed professional misconduct at a third facility on or between September 2017 to January 2018 in that the member:
- failed to keep records as required, contravened the standards of practice of the profession and engaged in disgraceful, dishonourable or unprofessional conduct by failing to document that the member administered a laxative to a patient in the medication administration record and/or failing to identify the type of laxative and dose administered in the progress notes; and/or
- contravened the standards of practice of the profession and engaged in disgraceful, dishonourable or unprofessional conduct, by stating to a colleague “your vagina is bulging out of your pants, that’s disgusting” or words to that effect; by stating to staff, including Personal Support Workers, “if any of you have a problem with me say it to my face” and/or asking staff to confront the member if they have an issue with the member and/or asking staff if they have a problem with the member, or words to that effect; by administering an unknown laxative to a patient without an authorizing order or applicable medical directive, documenting the administration of a PRN medication to promote bowel movement but failed to document what laxative was administered and what dose; by falsely telling one or more staff that the member worked for the College of Nurses of Ontario and/or that the member would audit them, or words to that effect; and/or on one or more occasions, raising the member’s voice at staff in the presence of patients and/or family members.

Details:
The following terms, conditions and limitations have been imposed on the member’s certificate of registration on an interim basis until the matter is disposed of by the Inquiries, Complaints and Reports Committee or the Discipline Committee:
- The Member must inform their employer of the reported concerns and that their conduct is being investigated by CNO;
- Within 14 days of commencing or continuing employment in a nursing position the Member must provide the Director, Professional Conduct with the following:
- Written notification of the name, address and telephone number of the employer;
- A signed statement from the employer confirming that the employer:
- Has been informed that a report of concerns has been sent to CNO and that CNO is investigating these concerns;
- Agrees to immediately notify the Director upon receipt of reasonable information that the Member has breached the standards of practice of the profession.
- The Member is restricted from practicing independently.
