Finding Details:
The member was found to have committed professional misconduct in that the member:
- contravened the standards of practice of the profession; failed to keep records as required; and engaged in disgraceful, dishonourable and unprofessional conduct in or around October 2015, by failing to have another nurse witness and sign a patient’s Individual Narcotic and Controlled Drug Count Sheet when wasting hydromorphone, and in or around July 2016, by failing to document changing another patient’s subcutaneous locks in the patient’s Medication Administration Record on three separate occasions; and
- contravened the standards of practice of the profession; falsified a record relating to her practice; signed or issued, in the member’s professional capacity, a document that the member knew or ought to have known contained a false or misleading statement; and engaged in disgraceful, dishonourable and unprofessional conduct between June 2016 and July 2016 by forging the signatures and/or initials of three of her nursing colleagues on the Individual Narcotic and Controlled Drug Count Sheets of two patients as witnesses to the waste and/or administration of hydromorphone and morphine sulfate on 12 occasions.

The Discipline Committee ordered that the member’s certificate of registration be suspended for 5 months; required the member to appear before the panel to be reprimanded; and imposed the following terms, conditions and limitations on the member’s certificate of registration:
- Must successfully complete specified remedial activities
- May not engage in independent nursing practice for a period of time
- Must inform employer(s) of results of discipline hearing
- Must inform College of all nursing employer(s) for a period of time
- Employer(s) to provide specified reports to CNO for a period of time
